About Us:

Harbour Hospice is the only specialist, palliative care provider for families living in the Hibiscus Coast, North Shore and Warkworth / Wellsford communities. We provide compassionate, free care working primarily with patients, families and carers in their homes, and also within our three hospice sites. Our bespoke services include medical care, spiritual support, physiotherapy, social activities and more, with an emphasis on helping people live every moment in whatever way is important to them. 

About the role:

This is a part time position, working 16 hours per week (0.4FTE) at our Hospice at Red Beach, supporting our lovely Hibiscus community.   This role sits in our Family Support Team which plays an integral role offering counselling, social work, cultural support, chaplaincy, life review and various support groups.

The key focus of the Social Work role will be to work within a multidisciplinary team improving and enhancing care and support of palliative care patients and their families.  The role will respond to the cultural and practical social needs of patients and advise them, and their family/whanau, on assistance and support available from statutory and community organisations.  You will also assist individuals and family/whanau to work on Advanced Care Planning (ACP) while maintaining autonomy and dignity.  Your focus will be to adjust care and support to meet the needs of patients and family/whanau.   

About you:

You will have demonstrated experience in social work , experience (or a genuine interest) in palliative care and knowledge of the holistic needs of patients and their families. 

Strong oral and written communication will be essential, as will the ability to work collaboratively and effectively in a multidisciplinary team environment and within the community. You will have:

  • Degree in Social Work
  • Registration with the Social Workers Registration Board (SWRB)
  • A current Annual Practicing Certificate
  • Demonstrated experience in working with and applying the principles of the Treaty of Waitangi
  • Experience with social work practice and working with a variety of people – particularly people dealing with grief and loss
  • Full current NZ Driver Licence
