0.9 FTE - 72 hrs per fortnight
Harbour Hospice is the only specialist, palliative care provider for families living in the Hibiscus Coast, North Shore and Warkworth / Wellsford communities. We provide compassionate, free care working primarily with patients, families and carers in their homes, and also within our three hospice sites. Our bespoke services include medical care, spiritual support, physiotherapy, social activities and more, with an emphasis on helping people live every moment in whatever way is important to them.
About the Role
The Clinical Nurse Specialist (CNS) will be based within our community nursing team, providing specialist nursing care and providing advice and support to contribute to the development of those within the team. This role will work to safeguard the current high standards of care we provide as well as continually looking to for improvements.
This role will be responsible for a clinical caseload, including the assessment of patients and family needs and reviewing of palliative care plans in conjunction with the Multidisciplinary Team (MDT).
About You As a Registered Nurse, you will have a post-graduate qualification and significant experience working in palliative care. With a focus on evidence-based practice you will contribute to advancing palliative care nursing practice.
You will be able to work independently and as a team player with a wide range of professionals across care settings of hospice, hospital and community organisations. As a CNS you will continue to develop your skills and knowledge and act as a role model, mentor and educator within the team.
You will have:
- Postgraduate diploma in specialty nursing practice/minimum of 5 years’ experience in specialist palliative care.
- Minimum Level 4 PDRP (Professional Development and Recognition Programme)
- Exceptional communication skills
- Evidence excellent skills in communication to individuals and as part of a team
- Current Full New Zealand driver licence
